Too Tired on GLP-1? Why Fatigue Happens and What to Do

If you are taking GLP-1 medications such as Wegovy, Ozempic or Mounjaro and you feel far more tired than expected, you are not imagining it.

For some people, the medication brings welcome appetite control but also an uncomfortable drop in energy.
You may feel:
  • flat
  • weak
  • foggy
  • less motivated
  • strangely “not yourself”
And that can be unsettling, especially if nobody warned you clearly enough.

The good news is that this is often understandable and manageable.

The more important question is not just “Is fatigue normal?” 
It is:

Why is it happening in your case — and what actually helps?

Can GLP-1 medication really make you tired?

​Yes.

In the current UK patient leaflet for Wegovy, “feeling weak or tired” is listed among the very common side effects, and the leaflet also warns that nausea, vomiting and diarrhoea can cause dehydration. I find my clients mention this very often to me.

The current Mounjaro leaflet lists fatigue as a common side effect and similarly warns about dehydration, especially when starting treatment.
Official UK guidance also notes that GLP-1 side effects are often worse when starting or increasing the dose and often improve over time. 

So yes, fatigue can happen.

But that does not mean you should ignore it.
​
Because “fatigue” on GLP-1 is often not just a random side effect.
It is often your body telling you that something in the bigger picture needs attention.

Why fatigue happens on Wegovy, Ozempic or Mounjaro​

​What I'm finding is that in real life, tiredness on GLP-1 medication is often driven by a combination of factors rather than one single cause.

1. You are eating far less than you realise

​This is one of the biggest reasons.

The medication reduces appetite, slows stomach emptying, and makes it easier to feel full quickly. That is part of how it works. But it also means some people slide into eating too little overall, especially in the first weeks or after dose increases.

Wegovy’s leaflet explicitly states that it works by helping you feel fuller and less hungry so that you eat less. 

In theory that sounds helpful.

In practice it can look like this:
  • half a yoghurt for breakfast
  • crackers or nothing for lunch
  • a few bites of dinner
  • coffee carrying the day
Then by late afternoon or evening, you feel:
  • exhausted
  • shaky
  • low
  • irritable
  • unable to think clearly
That is not weakness.
That is often under-fuelling.

2. Protein intake has dropped too low

​When people eat less on GLP-1, protein often drops first.

That matters because protein helps support lean mass, function, recovery and satiety. Recent expert guidance on nutrition priorities during GLP-1 therapy places adequate protein among the key practical priorities clinicians should focus on. 

If protein intake falls too low, you may notice:
  • more weakness
  • poorer recovery
  • lower resilience
  • more “washed out” energy

This is one reason I keep emphasising that eating less is not the same as eating well.

3. You may be dehydrated

​This is more common than people think, in fact it is probably one of the most common factors mentioned to me by my clients. 

The official leaflets for both Wegovy and Mounjaro warn that nausea, vomiting and diarrhoea can lead to dehydration and advise drinking enough fluids so this is important. 

Even without obvious vomiting or diarrhoea, dehydration can still creep in when:
  • appetite is low
  • you are eating less food overall
  • you are drinking less without noticing
  • you feel mildly nauseated and avoid both food and fluids

That can leave you feeling:
  • drained
  • headachy
  • dizzy
  • heavy-limbed
  • mentally foggy

4. Dose increases can hit your energy

​GLP-1 side effects often flare when treatment starts or when the dose goes up. The MHRA and your own side-effects content both reflect this dose-related pattern. 

This matters because many people think: 
“I was doing alright — why do I suddenly feel awful this week?”
​
Often the answer is:
  • your dose changed
  • your body is adjusting
  • your eating has not adapted properly
  • side effects have knocked your routine off balance

5. Low blood sugar may be relevant for some people

​For most people using GLP-1 medication purely for weight management, severe low blood sugar is not the main issue.
But if you also have diabetes, or if you are taking insulin or a sulphonylurea, the risk of hypoglycaemia is higher. Both Wegovy and Mounjaro patient information highlight this. 
If tiredness is coming with:
  • shakiness
  • sweating
  • confusion
  • sudden weakness
  • feeling faint
you should speak to your clinician promptly.

6. Fatigue affects behaviour as well as the body
​

​This part is often missed.

Once energy drops, everything gets harder:
  • shopping
  • cooking
  • drinking enough
  • moving your body
  • staying consistent
  • making sensible choices

So fatigue on GLP-1 is not just a physical symptom.

It can quietly unravel the very habits that are meant to support progress.

That is one reason people can feel confused:
 the medication is “working”, but they do not feel well.
And if that continues, motivation often drops next, this is a real 'risk' where a lot of your hard work can go to waste.

What GLP-1 fatigue often feels like in real life

​People do not always describe it as “fatigue”.

They say things like:
  • “I just feel wiped out”
  • “I can’t be bothered with anything”
  • “I’m not hungry, but I’m not functioning very well either”
  • “I thought this would make things easier, but I just feel flat”
  • “I’m losing weight, but I don’t feel stronger”

That pattern matters.

Because if the scale is moving but your energy, strength and daily function are slipping, the plan needs adjusting.

What actually helps

​This is the part most people are really looking for.

1. Stop aiming for “as little as possible”

​This is one of the most important mindset shifts.

Some people quietly treat appetite loss as a cue to eat almost nothing.

That usually backfires.

The goal is not to eat the tiniest amount you can survive on.
 The goal is to eat small amounts that still support your body well.

I like to think of it as:
  • less volume
  • more value

2. Prioritise protein early in the day

A protein-first approach is often one of the quickest ways to steady energy.

That might mean:
  • Greek yoghurt
  • eggs
  • cottage cheese
  • a protein smoothie
  • chicken soup
  • tuna
  • soft high-protein foods if appetite is low

If large meals feel unappealing, smaller protein-based meals usually work better than waiting until evening and then trying to “catch up”.

3. Take hydration more seriously than you think you need to

​Do not wait until you feel dramatically thirsty, I have found that when you are feeling thirsty there's a good chance you are already dehydrated..

Try to keep fluids steady through the day, especially if:
  • you feel sick
  • your mouth feels dry
  • you are constipated
  • your energy is dipping
  • you are drinking more caffeine than water

In some cases, especially if intake has been very low or there has been diarrhoea or vomiting, electrolytes may help — but persistent symptoms or significant dehydration need proper medical advice.

4. Eat regularly, even if appetite is low

​This does not mean forcing huge meals.

It means introducing enough structure that your body is not running on almost nothing.
For example:
  • breakfast with protein
  • a simple lunch, even if small
  • an easy protein snack
  • a lighter evening meal rather than skipping and crashing

You do not need perfection here.
You need enough consistency to stop the “nothing all day, then wiped out later” pattern.

5. Be careful with exercise when energy is low

Movement still matters.
But this is not the moment for punishing workouts if your intake is low and your energy is poor.

Gentle walking, strength-preserving movement, and realistic progression are usually better than trying to force intensity.

6. Look at the timing

​Ask yourself:
  • Did this start after a dose increase?
  • Is it worst on injection day or the day after?
  • Is it happening because nausea has reduced your food and fluids?

Those patterns help make sense of what is happening.
They also help you explain it more clearly if you need to speak to your prescriber.

When tiredness on GLP-1 is more concerning

Mild-to-moderate fatigue that improves as your body adjusts is one thing.

Fatigue that is severe, worsening, or accompanied by other symptoms is different.


Speak to your GP, prescriber, pharmacist or clinician if:
  • you cannot keep food or fluids down
  • you are vomiting repeatedly
  • you feel dizzy or faint
  • you have signs of dehydration
  • you have symptoms of low blood sugar
  • your fatigue is severe or not improving
  • you have severe stomach pain, especially if it is persistent or radiates to the back

The Wegovy and Mounjaro leaflets both highlight important red flags, including dehydration risk and symptoms that may suggest pancreatitis. The MHRA has also updated UK guidance for prescribers and patients around GLP-1 safety.

The bottom line

​If you are too tired on GLP-1, the answer is not always “just push through”.

Sometimes fatigue is your body’s way of telling you that:
  • intake has dropped too low
  • protein is not high enough
  • hydration is slipping
  • side effects are disrupting your routine
  • your current plan needs adjusting

The medication may be reducing appetite.
 But you still need a strategy.

Because long-term success is not just about eating less.
It is about losing weight without losing your energy, strength and ability to function well.

Is it normal to feel tired on Wegovy?
​

​Yes, tiredness or weakness can happen, especially early on or after dose increases, but it is still worth looking at food intake, hydration and the wider picture.

Why am I so tired on Mounjaro?
​

​Common reasons include eating too little, low protein intake, dehydration, side effects affecting routine, and adjustment after dose changes.

Will GLP-1 fatigue go away?
​

​Often it improves over time, especially if side effects settle and nutrition and hydration are addressed. If it is severe or persistent, speak to your clinician.

What should I eat if I feel weak on GLP-1?
​

​Usually small, protein-rich, easy-to-tolerate meals work better than skipping food and trying to catch up later.
