 Should You Exercise on Ozempic? A Safe Way to Start (Even If You’re Struggling)


Medications such as Ozempic (semaglutide), Wegovy (semaglutide) and Mounjaro (tirzepatide) can help reduce appetite and support  weight loss.
 
For many people this can feel like an important turning point.
 
But one important question often follows:
Should I be exercising while taking GLP-1 medication?
The short answer is yes — but exercise does not need to be intense, and it does not need to start with a gym membership.
 
For many people the most important goal is simply to begin moving regularly in ways that feel manageable and safe.
This is particularly important because weight loss, especially when appetite is reduced, can sometimes lead to loss of muscle as well as body fat.
 
Regular movement and simple strength exercises can help support:
  • muscle maintenance
  • mobility
  • energy levels
  • metabolic health
  • long-term weight maintenance

Why exercise matters 
​while taking GLP-1 medication

GLP-1 medications reduce appetite and often lead to reduced calorie intake.
 
That can help weight loss occur, but it also means the body may be losing tissue during that process.
Without physical activity, particularly some form of resistance movement, the body may lose muscle along with fat.
 
Maintaining muscle is important because muscle helps support:
  • strength and physical function
  • balance and injury prevention
  • metabolic health
  • long-term weight stability

Exercise during GLP-1 treatment is therefore less about burning calories and more about protecting your body while weight loss occurs.

Key Principles for Exercising 
While Taking GLP-1 Medication

When starting or returning to exercise while using medications such as Ozempic, Wegovy or Mounjaro, the most important principles are:

• Start small – gradual progress is safer and more sustainable
• Consistency matters more than intensity
• Protect muscle with some form of resistance exercise
• Listen to your body and avoid pushing through pain
• Choose activities you can maintain long-term

The goal is building movement into daily life in a way that feels achievable and sustainable.

If you have been sedentary start small

As mentioned above, many people starting GLP-1 medication have not exercised regularly for some time.
That is completely normal.

The most important principle is:
start small and build gradually.
Trying to do too much too quickly often leads to injury or discouragement.
A better approach is to focus on small, repeatable actions.

Simple movement counts

Exercise does not need to mean sport or gym workouts.
Many forms of movement support health, including:
  • walking
  • light cycling
  • swimming
  • gardening
  • housework
  • mobility exercises
  • stretching
 
Even small increases in daily movement can improve health. Park as far away from the shop whilst shopping, get off the bus a stop earlier. Those kind of small increases in activity all add up by the end of the week.

Protecting muscle during weight loss

Strength or resistance exercises can help maintain muscle while weight loss occurs.
This does not require heavy weights or advanced training.
 
Simple examples include:
  • chair sit-to-stand exercises
  • wall push-ups
  • resistance band exercises
  • light dumbbells
  • bodyweight movements
  • wheelchair bound people can exercise, they just need modifications
 
These exercises can often be performed at home and adjusted to suit different ability levels.
Even two short strength sessions per week can be beneficial.

Exercise if you have mobility limitations

Not everyone can perform traditional exercise.
 
Some people live with:
  • joint problems
  • chronic pain
  • disability
  • significant obesity
  • fatigue or medical conditions
 
Movement can still be adapted.
 
Examples include:
  • seated exercises
  • water-based exercise
  • resistance bands
  • physiotherapy-style mobility work
  • short supported walks
The key message is important:
Everyone can usually do something — the starting point simply needs to match the individual.

Signs you may be doing too much too quickly

When starting exercise after a long period of inactivity, it is important to progress gradually.
You may need to reduce intensity or rest if you experience:

• persistent joint pain
• excessive fatigue lasting several days
• dizziness during exercise
• unusual shortness of breath
• worsening mobility problems

Mild muscle soreness can be normal when beginning new activity, but sharp pain, persistent symptoms or feeling unwell should not be ignored.
If symptoms concern you, it is sensible to pause activity and seek advice from a healthcare professional.

Building long-term habits

One of the biggest benefits of starting exercise during GLP-1 treatment is that it helps build habits that support long-term health.
 
When medication reduces appetite and weight begins to change, it can be an ideal time to establish new routines.
 
These might include:
  • regular walks
  • short strength sessions
  • movement breaks during the day
  • active hobbies
 
The aim is not to become an athlete.
The aim is to create a life that includes movement naturally, and for the long term.

This type of structure is simply a starting point.
Progress should always be gradual and tailored to your own circumstances

Exercise readiness questionnaire

Before increasing physical activity, consider the following questions:
 
• Has your doctor ever advised caution with exercise because of a heart condition?
• Do you experience chest pain during activity?
• Do you feel dizzy or faint when exercising?
• Do you have joint problems that limit movement?
• Are you currently under treatment for a medical condition that affects physical activity?

 
If you answer yes to any of these questions, it may be sensible to discuss exercise plans with your GP or healthcare professional before increasing activity levels.

Before founding Health Renewed, I was a lifestyle coach, also working in health and fitness environments including delivering exercise-on-prescription programmes in partnership with GP surgeries.
 
These programmes helped individuals with little or no previous exercise experience begin moving safely and gradually.
 
The principles used here are the same: start gently, build slowly, and focus on sustainable habits rather than extreme exercise.
